The following information must be provided by the employee and the completed form should be returned to the Plan Administrator.

If coverage is offered to the dependant spouse name and date of birth must be given in the form.

After the form is completed in all respects one copy should be maintained by the employer and the employee with the original form sent to the boston mutual life insurance company for processing.

If there are additional coverages more copies need to be taken and forwarded to the respective administrator.

Insurance Refusal

The plan administrator has the responsibility to inform about the coverages to the participants who fall under the group plan.

The policy holders must also be informed about other features which they are eligible.
